You may authorize someone to act on your behalf or submit a request for your minor child. Requests may be
submitted no more than twice in a 12-month period.

We will verify your identity before processing your request. Our response time is typically within 45 days, with
a possible 45-day extension if needed. If we are unable to fulfill your request, we will explain why.
There is no fee unless the request is excessive or unfounded. In that case, a cost estimate will be provided
before completing the request.

Users Outside the U.S. and Canada
By using this site, you consent to your data being processed in the United States and other applicable
